The survey is completely voluntary and confidential. No one from TCBN will view your responses.
Your completed survey will automatically be sent to an evaluation consultant, who will consolidate answers from all the completed surveys.
The consolidated information will only be used by TCBN for research and program planning purposes. You will not be personally identified in any reports, and your answers will be combined with others in the final analysis. Your responses will not be shared with TCBN and will not impact your relationship with TCBN in the future.
This survey should take about 10 minutes to complete depending upon your responses. Please note that the deadline for completing this survey is October 16, 2020.
